Application for MembershipMembership of SPANZA is open to all anaesthetists holding a specialist qualification who have a particular interest in the anaesthesia care for children. The article of the constitution defining the three types of membership and the details of application for membership are contained in the membership section of the constitution.
Click here to download a pdf file of the membership application form which you can then print off and either post or fax to the address that appears on the bottom of the form.
SubscriptionThe annual subscription is currently AU$120+GST. There is also an initial joining fee of $20+GST. Often anaesthetists want to intially join following the Annual Meeting. In this case a member joining after the meeting would pay the initial joining fee of AU$20+GST and a reduced subscription for the remainder of the year, AU$ 50+GST. Once you are a member of SPANZA the invoice for subscription will be sent to you. The New Zealanders will receive this in early March and the Australians in May. This is to tie in with the financial year in each country.
Online payment If you are a member of SPANZA and wish to pay a subscription and have mislaid your invoice, please email the SPANZA Secretariat or call at +61 2 4973 6573. The secretariat can email you the link to pay by credit card on line, or send you the paperwork you need to send a cheque. Unpaid SubscriptionsIf a subscription is not paid, the member will be sent a reminder and another the following financial year. If the member remains in arrears the subscription will be written off as a bad debt. To rejoin, the anaesthetist will need to pay another joining fee and subscription.
Nomination to Executive CommitteeIf you would like to nominate another ordinary member of SPANZA for election to the Executive Committee, Click here to download a copy of the form which you can print out, complete and then fax to the secretary.
